What Is an Endpoint?
An endpoint is a pre-specified outcome measured to determine a drug’s effect. It must be: – Clearly defined before the trial begins – Measurable in a reliable, validated way – Clinically meaningful, it must matter to patients, physicians, or regulators
Primary vs. Secondary Endpoints
Every trial has one primary endpoint, the main outcome that determines trial success. Examples: – “Percentage of participants achieving IGA 0/1 (clear or almost clear skin) at week 16” (atopic dermatitis trial) – “Reduction in urine albumin-to-creatinine ratio at 52 weeks” (kidney disease trial) – “Change in EASI score from baseline at week 12” (eczema trial)
If the drug meets the primary endpoint with statistical significance, the trial is generally considered successful.
Secondary endpoints measure additional outcomes, safety, quality of life, time to response, durability of effect. They provide context but do not determine primary success.
Surrogate vs. Clinical Endpoints
A clinical endpoint directly measures patient outcomes, symptom severity, hospitalizations.
A surrogate endpoint is a measurable biological change believed to predict the clinical outcome. Surrogates enable faster trials but carry the risk that the surrogate does not perfectly track the actual outcome.
What This Means for Participants
Many procedures during study visits, scoring questionnaires, skin assessments, blood tests, are endpoint measurements. Your careful and honest participation in these assessments is not just procedural, it is scientifically essential.
