
Every acne breakout begins with a comedone. Understanding what a comedone is, why it forms, and how open and closed comedones differ is the foundation of understanding acne and choosing effective treatment.
What Is a Comedone?
A comedone is a follicular plug, a blockage in the opening of a hair follicle (pore). It forms when: 1. Sebaceous glands produce excessive sebum 2. The follicular lining sheds skin cells faster than they are cleared 3. Sebum and dead skin cells mix and compact within the follicular canal 4. The compacted material plugs the follicle
Bacteria, primarily Cutibacterium acnes (C. acnes), are present in comedones but are not the initial cause. The bacterium proliferates in the sebum-rich, low-oxygen environment of the plugged follicle, triggering the inflammation that converts a comedone into an inflammatory papule or pustule.
Open Comedones (Blackheads)
When the follicular plug reaches the surface, it is exposed to air. A common misconception is that blackheads are black because of dirt.
The black color is caused by: – Melanin oxidation, melanin pigment in expelled skin cells darkens when exposed to oxygen – Lipid oxidation, unsaturated fatty acids in sebum darken when exposed to air
Blackheads cannot be washed away, they are compact plugs, not surface dirt.
Closed Comedones (Whiteheads)
When the plug remains covered by a thin layer of skin, it is called a closed comedone. There is no air exposure, so no oxidation and no dark color. They appear as small, flesh-colored or slightly white raised bumps that are firm to the touch.
Treatment
Retinoids are the gold standard, adapalene, tretinoin, and tazarotene normalize follicular keratinization, preventing the shedding pattern that creates comedones. They are maintenance treatments that work over months, not days.
