What Is the Gut-Skin Axis?
The gut-skin axis refers to the bidirectional communication between the gastrointestinal microbiome, the gut immune system, and the skin. Proposed mechanisms include:
Immune regulation. The gut contains approximately 70% of the body’s immune cells. The gut microbiome plays a central role in training and regulating the immune system, including the Th1/Th2/Th17 balance that governs inflammatory skin disease.
Intestinal permeability. When the intestinal barrier is compromised, microbial products can translocate into the bloodstream, potentially triggering systemic inflammatory responses.
Short-chain fatty acids (SCFAs). Gut bacteria produce butyrate, acetate, and propionate from fermentation of dietary fiber. SCFAs have potent anti-inflammatory effects and may help regulate immune responses in the skin.
What the Research Shows
Atopic dermatitis: Infants and children with eczema have reduced gut microbiome diversity. Probiotic supplementation during pregnancy and early infancy shows modest preventive effects in some meta-analyses.
Psoriasis: Psoriasis is associated with gut dysbiosis and increased intestinal permeability. The association with inflammatory bowel disease, which shares genetic and immune pathways, supports genuine biological overlap.
