What Is Real-World Evidence and How Does It Differ From Clinical Trial Data?

The randomized controlled trial is the gold standard for demonstrating that a treatment works. But clinical trials, by their nature, are conducted in highly controlled conditions that can differ substantially from real-world clinical practice. Real-world evidence (RWE) fills the gaps that trials leave behind.

What Is Real-World Evidence?

Real-world evidence is clinical evidence about the usage, potential benefits, or risks of a medical product derived from analysis of real-world data (RWD). RWD includes:

  • Electronic health records (EHRs), data from routine clinical care
  • Insurance claims data, records of diagnoses, procedures, and prescriptions
  • Patient registries, organized databases tracking patients with specific conditions
  • Patient-generated data, wearable devices, apps, and patient-reported outcomes outside of a trial
  • Pragmatic trials, trials conducted in routine clinical settings rather than highly controlled research environments

How RWE Differs From Clinical Trial Data

Population. Clinical trials enroll selected patients meeting strict criteria. RWE captures diverse, real-world patients, older adults, those with multiple conditions, those on complex medication regimens, who are often excluded from trials.

Setting. Trials use controlled, monitored environments. RWE reflects care as actually delivered.

Duration. Trials typically run months to a few years. RWE can capture outcomes over decades.

Endpoints. Trials focus on pre-specified endpoints. RWE can capture rare outcomes, long-term safety signals, and outcomes that are difficult to measure in controlled conditions.

How the FDA Uses RWE

The FDA increasingly accepts RWE to: – Support new indications for approved drugs – Satisfy post-approval study requirements – Assess long-term safety beyond the trial period – Inform labeling updates

Why Both Are Needed

Clinical trials establish causality, they demonstrate that the treatment, not confounding factors, produced the outcome. RWE supplements this with generalizability, it shows how the treatment performs in the full spectrum of patients who will actually use it.
