PCOS and Acne: Understanding the Hormonal Connection

What Is PCOS?

Androgens, particularly testosterone and its active form DHT, are the primary hormonal drivers of sebaceous gland activity. Elevated androgens in PCOS:

  • Increase sebum production
  • Promote follicular keratinization (the cellular process that creates comedones)
  • Create the sebum-rich environment in which C. acnes proliferates

The result is acne resistant to standard treatments because its root cause is systemic and hormonal.

Recognizing PCOS-Associated Acne

  • Persistent or new-onset acne in women aged 25+
  • Predominantly lower face, jawline, and chin location
  • Premenstrual worsening
  • Concurrent hirsutism, irregular periods, or difficulty losing weight
  • Inadequate response to standard acne treatments

Treatment Approaches

  • Spironolactone, highly effective for hormonal acne; blocks androgen receptors
  • Combined oral contraceptives, with anti-androgenic progestins
  • Metformin, improves insulin sensitivity, reduces androgen levels indirectly
  • Topical retinoids, address the local follicular component
