Dupilumab, sold under the brand name Dupixent, is widely regarded as the most significant advance in atopic dermatitis treatment in a generation. Since its FDA approval in 2017, it has transformed outcomes for hundreds of thousands of patients with moderate-to-severe eczema.
What Is Dupilumab?
Dupilumab is a monoclonal antibody that specifically targets two key inflammatory signaling molecules: IL-4 and IL-13. By blocking the receptor shared by these two cytokines, it suppresses the Th2 inflammatory pathway central to atopic dermatitis pathology.
This targeted approach is fundamentally different from broad immunosuppressants, dupilumab modulates a specific inflammatory axis rather than suppressing the immune system globally.
FDA Approvals
Dupilumab is approved for: – Moderate-to-severe atopic dermatitis in adults (2017) – Atopic dermatitis in children aged 6 months and older (expanded 2019–2023) – Moderate-to-severe asthma – Chronic rhinosinusitis with nasal polyposis – Prurigo nodularis
How Is It Administered?
Dupilumab is administered by subcutaneous injection, a self-administered shot given under the skin every 2 weeks. After initial loading doses, the schedule becomes every 2 or 4 weeks depending on age, weight, and indication. Many patients self-inject at home after training.
What Results Can Patients Expect?
In clinical trials, approximately 60–70% of adults achieved IGA 0/1 (clear or almost clear skin) at 16 weeks. Most patients see meaningful improvement within 4–8 weeks.
Common side effects include injection site reactions and conjunctivitis, the latter usually managed with lubricating eye drops.
Beyond Dupilumab
Several newer biologics, tralokinumab, lebrikizumab, and nemolizumab, are now approved or in late-stage development. Clinical trials continue to evaluate new combinations and indications.
