If you are enrolled in an atopic dermatitis (eczema) clinical trial, one of the most important assessments you will undergo at each visit is the EASI evaluation.
What Is the EASI Score?
EASI stands for Eczema Area and Severity Index. It is the gold-standard measure of atopic dermatitis severity, used in regulatory submissions globally. The score ranges from 0 (clear) to 72 (maximum severity).
How Is EASI Calculated?
A trained investigator evaluates four body regions: head/neck, trunk, upper extremities, and lower extremities. For each region, four signs are scored:
- Erythema (redness)
- Edema/papulation (swelling)
- Excoriation (scratch marks)
- Lichenification (skin thickening)
Each sign is scored 0 to 3. The area affected in each region is also scored. These are combined using a weighted formula.
What the Numbers Mean
- Clear/Almost Clear: EASI 0 to 1
- Mild: EASI 1 to 7
- Moderate: EASI 7 to 21
- Severe: EASI 21 to 50
- Very Severe: EASI above 50
Common Clinical Trial Endpoints
- EASI-50: 50% reduction from baseline, minimum standard for meaningful improvement
- EASI-75: 75% reduction, considered a good treatment response
- EASI-90: 90% reduction, near-complete clearance
The EASI is objective, reproducible, and recognized by the FDA and EMA as a primary endpoint in atopic dermatitis trials.
