Black Patients and Clinical Trial Access: Addressing Historical Barriers

Black Americans participate in clinical trials at rates significantly below their population representation, a disparity driven by well-founded historical mistrust, structural access barriers, and systemic inequities in healthcare. Addressing these barriers is both an equity goal and a medical necessity.

The Historical Context

Mistrust of medical research among Black Americans is not irrational, it is a rational response to documented history.

The Tuskegee Study (1932–1972), the U.S. Public Health Service deliberately withheld penicillin from Black men with syphilis to study the disease’s natural progression. The study ran for 40 years. Its legacy created justifiable skepticism that persists today.

Additional historical abuses, forced sterilizations, non-consensual experiments on enslaved people, unequal access to care, compound this mistrust.

The Consequences of Underrepresentation

Drug dosing and safety. Pharmacogenomic variants that affect drug metabolism differ in frequency across populations. Without adequate representation in trials, standard dosing may be less effective or less safe for underrepresented groups.

APOL1 and kidney disease. APOL1 high-risk gene variants are carried by approximately 13% of Black Americans and drive dramatically elevated kidney disease risk. Without Black participation in kidney disease trials, treatments targeting these variants cannot be properly developed.
