
The randomized controlled trial, the RCT, is often called the gold standard of medical evidence. Every major clinical guideline and drug approval is ultimately built on RCT data.
The Core Idea
A randomized controlled trial compares two or more treatments by: 1. Randomly assigning participants to one group or another 2. Controlling for confounding variables by distributing unknown and known factors equally across groups 3. Comparing outcomes between groups to determine which treatment produced better results
The random assignment is the critical element. Without it, sicker patients might cluster in one group, making it impossible to attribute outcome differences to the treatment.
Why “Controlled”?
Every trial needs a control group, a benchmark against which the experimental treatment is measured. The control is typically: – Placebo, an inert substance that looks identical to the active treatment – Active comparator, the current standard of care – Different dose, comparing two doses of the same drug
Without a control group, improvements in a treated group could simply reflect natural disease fluctuation or the placebo effect.
What Randomization Accomplishes
Properly executed randomization distributes both known and unknown confounding factors across treatment groups, even factors the researchers do not know to measure. This allows researchers to attribute outcome differences to the treatment rather than pre-existing differences between groups.
