What Is the Skin Microbiome?

The skin microbiome refers to the community of microorganisms on the skin surface and in hair follicles and sebaceous glands. Composition varies dramatically by body site: – Dry areas, dominated by Staphylococcus and Corynebacterium species – Moist areas, Corynebacterium and Brevibacterium – Sebaceous areas (face, scalp, chest), dominated by Cutibacterium and Malassezia

The Skin Microbiome and Atopic Dermatitis

One of the most significant findings is the consistent overgrowth of Staphylococcus aureus on the skin of people with atopic dermatitis, particularly during flares.

S. aureus disrupts the skin barrier and triggers inflammation through multiple mechanisms: – Secretes toxins that act as superantigens, massively amplifying immune responses – Produces proteases that degrade filaggrin (a protein critical for skin barrier function) – Activates the same IL-4 and IL-13 pathways targeted by dupilumab

This bidirectional relationship, where atopic dermatitis creates an environment promoting S. aureus, which then worsens atopic dermatitis, is an active area of research.

The Microbiome and Acne

Acne was historically attributed to C. acnes overgrowth. Current understanding is more nuanced: it is not the abundance of C. acnes but the specific strains present that correlate with acne risk. Acne-prone skin contains different C. acnes strains compared to healthy skin.

Emerging Therapeutic Approaches

  • Topical applications of beneficial bacteria (Roseomonas mucosa for eczema)
  • Phage therapy to selectively reduce pathogenic bacterial strains
